Request Boarding Dates
Use this form to request boarding dates. Please also click here to download our boarding care & authorization form. Please print one for each pet you'll be boarding with us, and fill them out before you come in.
All animals staying at Ahwatukee Commons Veterinary Hospital must be current on their vaccinations. If your pet is not up to date, we can take care of it when they come in.
Dogs must be current on their Distemper/Parvo, Rabies and Bordetella (kennel cough) vaccines. The Bordetella must have been administered in the last 6 months (depending on manufacturer guidelines and route given). We also require that all dogs and cats be treated with a flea/tick treatment. Frontline can be applied 48 hours before the boarding appointment by a member of our staff, or a flea/tick treatment can be administered when the pet is dropped off.
Cats and ferrets must be current on their Distemper and Rabies vaccines.
